== History == Etoperidone was discovered by scientists at Angelini, who also discovered trazodone. Its development names have included ST-1191 and McN-A-2673-11. The INNTooltip International Nonproprietary Name etoperidone was proposed in 1976 and recommended in 1977. The drug was given brand names in Spain (Centren (Esteve) and Depraser (Lepori)) and Italy (Staff (Sigma Tau)) and was also given the brand names Axiomin and Etonin, but it is not entirely clear if it was actually marketed; the Pharmaceutical Manufacturing Encyclopedia provides no dates for commercial introduction. According to Micromedex's Index Nominum: International Drug Directory, etoperidone was indeed previously marketed in Spain and Italy.
=== Eukaryotic mRNA turnover === Inside eukaryotic cells, there is a balance between the processes of translation and mRNA decay. Messages that are being actively translated are typically associated with cap-binding and translation initiation factors and with poly(A)-binding protein, which can antagonize deadenylation and decapping and help protect mRNA ends from decay machinery. The balance between translation and decay is reflected in the size and abundance of cytoplasmic structures known as P-bodies. The poly(A) tail of the mRNA is shortened by specialized exonucleases that are targeted to specific messenger RNAs by a combination of cis-regulatory sequences on the RNA and trans-acting RNA-binding proteins. Poly(A) tail removal is thought to disrupt the circular structure of the message and destabilize the cap binding complex. The message is then subject to degradation by either the exosome complex or the decapping complex. In this way, translationally inactive messages can be destroyed quickly, while active messages remain intact. As a second lieutenant in 1966, Noriega spent many months taking courses at the School of the Americas. The school was located at the United States Army's Fort Gulick in the Panama Canal Zone. Journalist John Dinges has suggested that Torrijos sent Noriega to the school to help him "shape up" and live up to Torrijos's expectations. Despite performing poorly in his classes, he was promoted to the rank of first lieutenant in 1966, and Torrijos found him a job as an intelligence officer in the "North Zone" of the National Guard. Shortly afterward, he returned to the School of the Americas for more training. At the school, Noriega participated in courses on infantry operations, counterintelligence, intelligence, and jungle warfare. He also took a course in psychological operations at Fort Bragg in North Carolina. Noriega's job required him to penetrate and disrupt the trade unions that had formed in the United Fruit Company's workforce, and he proved adept at this work. His new superior officer Boris Martínez was a fervent anti-communist, and enforced strict discipline on Noriega. Reports have suggested that he continued to pass intelligence to the U.S. during this period, about the plantation workers' activities. In 1967 the administration of U.S. President Lyndon B. Johnson concluded that Noriega would be a valuable asset, as he was a "rising star" in the Panamanian military. Later, as the de facto leader of Panama, Noriega maintained a close relationship with the School of the Americas, partly due to the school's presence in Panama.

== Medical uses == The compound is indicated for adults with type 2 diabetes as an adjunct to diet and exercise to improve glycemic control. Dulaglutide is not indicated in the treatment of subjects with type 1 diabetes or patients with diabetic ketoacidosis because these problems are the result of the islet cells being unable to produce insulin and one of the actions of dulaglutide is to stimulate functioning islet cells to produce more insulin. Dulaglutide can be used either stand-alone or in combination with other medicines for type 2 diabetes, in particular metformin, sulfonylureas, thiazolidinediones, and insulin taken concomitantly with meals. The medication's phase 3 clinical trial program demonstrated reductions in hemoglobin A1c of approximately 1% with the 0.75 mg and 1.5 mg doses of the medication, along with approximately 5 pounds of weight loss on average. The higher 3.0 mg and 4.5 mg doses that were approved in 2020 demonstrated hemoglobin A1c reductions closer to 1.5% and slightly more weight loss. An alternate study conducted by REWIND had enrolled a lower-risk population consisting of 9901 patients, two-thirds had no prior cardiovascular events. These patients were randomized to dulaglutide 1.5mg weekly versus a placebo group for a median of 5.4 years. As a result of this study, Dulaglutide had reduced MACE by 12% (HR 0.88, 95% Cl 0.79-0.99) and had produced a 15% relative risk reduction in the composite renal outcome.
