=== Amyotrophic lateral sclerosis === Amyotrophic lateral sclerosis (ALS), also known as Lou Gehrig's Disease, is a progressive neurodegenerative disease that affects nerve cells in the brain and the spinal cord. Motor neurons reach from the brain to the spinal cord and from the spinal cord to the muscles throughout the body. The progressive degeneration of the motor neurons in ALS eventually leads to their death. When the motor neurons die, the ability of the brain to initiate and control muscle movement is lost. With voluntary muscle action progressively affected, patients in the later stages of the disease may become totally paralyzed. In January 2007, INSMED announced that the Italian Ministry of Health requested INSMED corporation to make Iplex available to treat Italian patients sufferings from ALS. IGF-1, the main component of Iplex, was the subject of a recent clinical trial in ALS. It involved 330 people with ALS from 20 ALS treatment centres across the United States. The drug was injected under the skin (subcutaneous delivery) in a randomized double-blinded placebo-controlled trial – this is the gold standard way of conducting a clinical trial. At the end of the two-year treatment period, there were no differences between people with ALS who received IGF-1 and those who received placebo in muscle strength, the need for a tracheostomy for breathing, or survival, indicating that IGF-1 provided patients no benefit. === Subclinical hypothyroidism === There is no evidence of a benefit from treating subclinical hypothyroidism in those who are not pregnant, and there are potential risks of unnecessary overtreatment. Untreated subclinical hypothyroidism may be associated with a modest increase in the risk of coronary artery disease when the TSH is over 10 mIU/L. There may be an increased risk for cardiovascular death. A 2007 review found no benefit of thyroid hormone replacement except for "some parameters of lipid profiles and left ventricular function". There is no association between subclinical hypothyroidism and an increased risk of bone fractures, nor is there a link with cognitive decline. American guidelines recommend that treatment should be considered in people with symptoms of hypothyroidism, detectable antibodies against thyroid peroxidase, a history of heart disease, or are at an increased risk for heart disease if the TSH is elevated but below 10 mIU/L. American guidelines further recommend universal treatment (independent of risk factors) in those with TSH levels that are markedly elevated; above 10 mIU/L because of an increased risk of heart failure or death due to cardiovascular disease. NICE recommends that those with a TSH above 10 mIU/L should be treated in the same way as overt hypothyroidism. Those with an elevated TSH but below 10 mIU/L who have symptoms suggestive of hypothyroidism should have a trial of treatment but intend to stop this if the symptoms persist despite normalization of the TSH.

=== Funding trajectory === Since 1999, the division's funding increased more than six-fold. By 2012, OAM and NCCAM spent a cumulative $1.6 billion in grant funding. Between 1999 and 2009, NCCAM supported approximately 50% of the National Cancer Institute spending on CAM, with the total amount spent on CAM during that time frame $2.856 billion. The NCCIH budget for 2005 was $123 million. For fiscal year 2009 (ending September 30, 2009), it was $122 million.
