=== Development of synthetic AAS === The development of muscle-building properties of testosterone was pursued in the 1940s, in the Soviet Union and in Eastern Bloc countries such as East Germany, where steroid programs were used to enhance the performance of Olympic and other amateur weight lifters. In response to the success of Russian weightlifters, the U.S. Olympic Team physician John Ziegler worked with synthetic chemists to develop an AAS with reduced androgenic effects. Ziegler's work resulted in the production of metandienone, which Ciba Pharmaceuticals marketed as Dianabol. The new steroid was approved for use in the U.S. by the Food and Drug Administration (FDA) in 1958. It was most commonly administered to burn victims and the elderly. The drug's off-label users were mostly bodybuilders and weight lifters. Although Ziegler prescribed only small doses to athletes, he soon discovered that those having used metandienone developed enlarged prostates and atrophied testes. AAS were placed on the list of banned substances of the International Olympic Committee (IOC) in 1976, and a decade later, the committee introduced "out-of-competition" doping tests because many athletes used AAS in their training period rather than during competition.
Bacitracin is a polypeptide antibiotic. It is a mixture of related cyclic peptides produced by Bacillus licheniformis bacteria, that was first isolated from the variety "Tracy I" (ATCC 10716) in 1945. These peptides disrupt Gram-positive bacteria by interfering with cell wall and peptidoglycan synthesis. Bacitracin is primarily used as a topical preparation, as it can cause kidney damage when used internally. It is generally safe when used topically, but in rare cases may cause hypersensitivity, allergic or anaphylactic reactions, especially in people allergic to neomycin. In 2023, it was the 323rd most commonly prescribed medication in the United States, with more than 100,000 prescriptions.
=== Legal status === Around the world, codeine is, contingent on its concentration, a Schedule II and III drug under the Single Convention on Narcotic Drugs. In Australia, Canada, New Zealand, Sweden, the United Kingdom, the United States and many other countries, codeine is regulated under various narcotic control laws. In some countries, it is available without a medical prescription in combination preparations from licensed pharmacists in doses up to 20 mg, or 30 mg when sold combined with 500 mg paracetamol. As of 2015, of the European Union member states, 11 countries (Bulgaria, Cyprus, Denmark, Estonia, Ireland, Latvia, Lithuania, Malta, Poland, Romania, and Slovenia) allow the sale of OTC codeine solid dosage forms.
== Anaphylaxis during general anaesthesia == Administration of pholcodine causes production of antibodies linked with fatalities during surgery, when essential neuromuscular blocking agents (NMBAs) are administered to prevent patient movement under general anaesthesia. These antibody levels gradually fall to low levels several years after last dose of pholcodine. However, the presence of these antibodies causes a 300-fold increase in risk of anaphylaxis during anaesthesia. The link was suspected when neighbouring Norway and Sweden were found to have tenfold differences of surgical anaphylaxis deaths. Sweden had no products approved containing pholcodine, whereas 40% of the population in Norway had consumed the single approved pholcodine product. Norway withdrew pholcodine from the market in 2007, and the prevalence of anti-suxamethonium antibodies fell by over 80% in two years. A corresponding fall in anaesthesia deaths followed. A similar disparity exists between NMBA anaphylaxis rates in Australia, where pholcodine consumption is high and the US, where pholcodine is banned. In the US, anaphylaxis rates are so low that some anaesthetists question the existence of such reactions to NMBAs. Conversely, Australian anaesthetists have requested a ban on pholcodine due to the high anaphylaxis rate in the country. However, the Therapeutic Goods Administration declined the request in January 2015, pending further reviews to follow. In February 2023, the Therapeutic Goods Administration reversed its previous decision and banned products containing pholcodine.

== Nerve supply == Heart rate depends on the rate at which the sinoatrial node produces action potentials. At rest, the human heart rate is between 60 and 100 beats per minute. This is a result of the activity of two sets of nerves, one acting to slow down action potential production (these are parasympathetic nerves) and the other acting to speed up action potential production (sympathetic nerves). Modulation of heart rate by ANS is carried by two types of channel: Kir and HCN (members of the CNG gated channels). The sympathetic nerves begin in the thoracic region of the spinal cord (in particular T1-T4). These nerves release a neurotransmitter called noradrenaline (NA). This binds to a receptor on the SA node membrane, called a beta-1adrenoceptor. Binding of NA to this receptor activates a G-protein (in particular a Gs-Protein, S for stimulatory) which initiates a series of reactions (known as the cAMP pathway) that results in the production of a molecule called cyclic adenosinemonophosphate (cAMP). This cAMP binds to the HCN channel (see above). Binding of cAMP to the HCN increases the flow of Na+ and K+ into the cell, speeding up the pacemaker potential, so producing action potentials at a quicker rate and increasing heart rate. An increase in heart rate is known as positive chronotropy. The parasympathetic nerves supplying the SA node (in particular the Vagus nerves) originate in the brain. These nerves release a neurotransmitter called acetylcholine (ACh). ACh binds to a receptor called an M2 muscarinic receptor, located on the SA node membrane.
In March 2018 Sara Rojas and her team published their research on drug incorporation and delivery with various biocompatible MOFs other than CD-MOF-1 through simulated cutaneous administration. The group studied the loading and release of ibuprofen (hydrophobic) and aspirin (hydrophilic) in three biocompatible MOFs (MIL-100(Fe), UiO-66(Zr), and MIL-127(Fe)). Under simulated cutaneous conditions (aqueous media at 37 °C) the six different combinations of drug-loaded MOFs fulfilled "the requirements to be used as topical drug delivery systems, such as released payload between 1 and 7 days" and delivering a therapeutic concentration of the drug of choice without causing unwanted side effects. The group discovered that the drug uptake is "governed by the hydrophilic/hydrophobic balance between cargo and matrix" and "the accessibility of the drug through the framework". The "controlled release under cutaneous conditions follows different kinetics profiles depending on: (i) the structure of the framework, with either a fast delivery from the open structure MIL-100 or a slower drug release from the narrow 1D pore system of MIL-127 or (ii) the hydrophobic/hydrophilic nature of the cargo, with a fast (Aspirin) and slow (Ibuprofen) release from the UiO-66 matrix." Moreover, a simple ball milling technique is used to efficiently encapsulate the model drugs 5-fluorouracil, caffeine, para-aminobenzoic acid, and benzocaine. Both computational and experimental studies confirm the suitability of [Zn4O(dmcapz)3] to incorporate high loadings of the studied bioactive molecules.

=== Italy === In Italy, after completing a cycle of 6 years of studies, students get the title of "Medico generico" (generic physician), and are authorized to limited activities, like: teach first aid practices, act as temporary substitute of a primary care physician or issue medical certificates for sports purposes. == Side effects == Frequently (≥1% to <10%) reported adverse reactions with opipramol, especially at the beginning of the treatment, include fatigue, dry mouth, blocked nose, hypotension, and orthostatic dysregulation. Rarely (≥0.01% to <0.1%) reported adverse reactions include excitation, headache, paresthesia especially in elderly patients, restlessness, sweating, sleep disturbances, edema, galactorrhea, urine blockage, nausea and vomiting, fever, collapse conditions, stimulation conducting disturbances, intensification of present heart insufficiency, blood profile changes particularly leukopenia, confusion, delirium, stomach complaints, taste disturbance, and paralytic ileus especially with sudden discontinuation of a longer-term high-dose therapy. Very rarely (<0.01%) reported adverse reactions include seizures, motor disorders (akathisia, dyskinesia, ataxia), polyneuropathy, glaucoma, anxiety, hair loss, agranulocytosis, severe liver dysfunction after long-term treatment, jaundice, and chronic liver damage.
=== Synergism === Several antibiotics have been tested as partner drugs of oritavancin. Among these "companions" drugs, fosfomycin displayed (in vitro and in vivo) synergistic activity when administered together with oritavancin against VRE strains (both vanA and vanB), including biofilm-producing isolates. This synergistic action has also been proposed for the prevention of vascular graft infections by impregnating prostheses with a combination of oritavancin and fosfomycin.
=== Serotonin receptors === The serotonin receptors are located on the cell membrane of nerve cells and other cell types in animals, and mediate the effects of serotonin as the endogenous ligand and of a broad range of pharmaceutical and psychedelic drugs. There are currently 14 known serotonin receptors, including the serotonin 5-HT1 (1A, 1B, 1D, 1E, 1F), 5-HT2 (2A, 2B, 2C), 5-HT3, 5-HT4, 5-HT5 (5A, 5B), 5-HT6, and 5-HT7 receptors. Except for the serotonin 5-HT3 receptor, a ligand-gated ion channel, all other 5-HT receptors are G-protein-coupled receptors (also called seven-transmembrane, or heptahelical receptors) that activate an intracellular second messenger cascade. === Endoscopy === After Edlich learned about endoscopes in Asia, he received an endoscope for gastroscopic visualization of the patient's stomach. As a surgical resident, Edlich was the first physician to do a gastroscopy at the University of Minnesota Medical Center. Edlich's picture was featured on the cover of Postgraduate Medicine, the Journal of Applied Medicine, in November 1968. His experiences with gastroscopy led to the first minimally invasive surgical procedure at the University of Minnesota Hospital, an endoscopic gastrostomy. Edlich soon realized the limitations of the thick, narrow-diameter latex Ewald tubes that were being used to evacuate blood clots from the patient's stomach before endoscopic examination. == Current food inflation == On 16 February 2025, Mohsen Baghaei, a workers' representative on the Supreme Labor Council, warned of steep price hikes in essential goods over the past two months, with cooking oil up 40%, rice nearly doubling, and staples like potatoes and onions also surging. He estimated that the average worker’s cost-of-living basket has risen over 30% in this period, jumping from 300 million rials ($325) to 380 million rials ($413). According to Iran international as of February 2025 the price of one kg of kiwi equals 2% of the average income. On 23 February 2025, a government agency announced an annual rate of 35%. However, labor groups and analysts argue that the government minimizes negative economic news, implying that the actual inflation rate is considerably higher. The Iranian rial has depreciated by more than 50% over the past six months, fueling expectations of even higher inflation.
