=== Columbia University: 1933-1934 === In April 1933, Schoenheimer emigrated to the United States in response to the Nazi regime's policy for the dismissal of Jewish faculty in universities. He was offered work at Columbia University as an assistant professor, where he continued his research on metabolism and cholesterol synthesis, alongside Walter M. Sperry and David Rittenberg. In 1933, Germany entered a political crisis and saw the rise of Hitler and the Nazi Party, which led Schoenheimer into emigrating to the United States. Schoenheimer had Jewish heritage which he was conscious of despite his Family having converted to Christianity. Schoenheimer had joined a Jewish Youth Movement and was actively involved in the Zionist Organisation following WW1. Due to the situation in Germany at the time and the Nazi Policy dismissing Jewish faculty, Schoenheimer was unlikely to remain living in Germany. Ludwig Kast was informed of this situation and contacted U.S universities, Cornell and Columbia on behalf of Schoenheimer. Hans T. Clarke, the Chairman of the Biological Chemistry Department at the University of Columbia, invited Schoenheimer to work at the university. He began work in the Department of Biological Chemistry as a research assistant. The Josiah Macy Foundation provided his salary and research support whilst he worked at Columbia. ==== Mechanism ==== There are three types of opioid receptors: mu (μ-opioid receptors), delta, and kappa (κ-opioid receptor). Endogenous opioids (enkephalins, dynorphin, endorphin) do not bind specifically to any particular opioid receptor. Receptor binding of the opioid causes a cascade leading to the channel opening and hyperpolarization of the neuron. The opioid receptors have the following channel types: mu, K+ channel; l delta, K+ channel; kappa, Ca2+ channel. Hyperpolarization can lead to post-synaptic neural inhibition and presynaptic inhibition of neurotransmitter release. Post-synaptic neural inhibition can reduce analgesia and central hyperactivity may reduce its efficacy. The mechanism of kappa receptors is slightly different from mu and delta, in that Ca2+ channels close instead of K+ channels, and K+ channels open in mu and delta.

Acute ischemic strokes start when there is reduced blood flow, often caused by an occlusion, to part of the brain. Even if an occlusion causes a complete blockage of a major artery, there is typically still some blood flow downstream of the blockage through collateral blood vessels. With reduced blood flow, there is reduced oxygen supply, and to compensate the tissue goes through anaerobic metabolism which is much less efficient. If anaerobic metabolism does not provide enough energy, there is energy failure, followed by ion imbalances. Afterwards, the pathophysiology gets complicated and there are thought to be at least eight pathways of tissue damage. By targeting processes near the top of the top of the chain of events, problems further down the chain of events can be avoided. For example, the drug tPA and mechanical thrombectomy devices all target the occlusion which is at the top of the chain of events, and have achieved FDA approval. The next step in the chain of events is hypoxia, and some oxygen delivery drugs have shown strong effects in animal studies, as shown in the table below. If processes further down the chain of events get targeted, there may be many simultaneous problems and the effect of a single therapy may be less, so there may be benefit to using multiple drugs in combination to treat multiple pathways.

Trazodone is a mixed agonist and antagonist of various serotonin receptors, antagonist of adrenergic receptors, weak histamine H1 receptor antagonist, and weak serotonin reuptake inhibitor. More specifically, it is an antagonist of 5-HT2A and 5-HT2B receptors, a partial agonist of the 5-HT1A receptor, and an antagonist of the α1- and α2-adrenergic receptors. It is also a ligand of the 5-HT2C receptor with lower affinity than for the 5-HT2A receptor. However, it is unknown whether trazodone acts as a full agonist, partial agonist, or antagonist of the 5-HT2C receptor. Trazodone is a 5-HT1A receptor partial agonist similarly to buspirone and tandospirone but with comparatively greater intrinsic activity. A range of weak affinities (Ki) have been reported for trazodone at the human histamine H1 receptor, including 220 nM, 350 nM, 500 nM, and 1,100 nM.
